Veterans Resource Center

The Veterans Resource Center connects Veterans with the services they need –The VRC is focused on the overall wellness and reintegration of Military Veterans of all eras. With a primary focus on housing assistance, case management, and outreach, we take a holistic approach to addressing the individual needs of each Veteran. We invite all community partners to utilize the VRC as we combine efforts and collaborate to help those we serve. The VRC serves as a single-entry point to provide the help Veterans need by forming a coordinated network of community partners. ​The Veterans Service Center is located in Medford, OR but can serve any veterans in the catchment area of the VA SORCC (Jackson, Josephine, Klamath, and Lake counties). We work with community partners within the region to provide referral/information as applicable.

Veterans Case Management

  • ColumbiaCare is a contracted provider with the Veterans Administration to provide Veterans Affairs Supportive Housing (VASH) Case Management Services in Washington. These services are provided by a dedicated team of mental health and housing professionals who work in partnership with the VA and other community agencies to help connect Veterans with qualified housing, as well as Case Management Services to assist them in maintaining that housing as they stabilize in other areas of their lives. ColumbiaCare's role in helping the Veteran during their reintegration and recovery process may include assisting with obtaining benefits, financial management, independent living skills, interfacing with landlords, accessing public transit and services, maintaining sobriety and improving mental health. (Seattle Metro Area, American Lakes & Snohomish)

  • ​Our Grant and Per Diem (GPD) Housing Case Management program provides wraparound supportive services to Veterans who were previously homeless or at-risk of losing their housing. The GPD Case Management grants improve housing retention among formerly homeless Veterans by providing an array of individualized services that promote housing stability. When Veterans transition from homelessness to permanent housing, some may need help readjusting to the routines and responsibilities of managing their household affairs. The GPD Case Management can help ensure that formerly homeless Veterans have the tools to remain in stably housed and successfully reintegrate back into their communities. Eligibility criteria for the GPD Case Management program: is: The Veteran is recently graduated from GPD or HCHV Housing, a Veteran is newly housed, or a Veteran is at risk of losing housing without other Case Management support. Program admission is contingent upon final approval after screening from GPD Program Staff. (Jackson County)

  • SSVF promotes housing stability among low-income Veteran families who reside in or are transitioning to permanent housing. We do this through the provision of case management and temporary financial support. The program provides rapid rehousing and homeless prevention services through evidence-based practices and case management strategies in order to achieve housing stability. Case Managers provide eligible Veteran families with outreach, case management, and assistance in obtaining VA and other benefits, which may include: health care navigation services, daily living services, personal financial planning services, transportation services, fiduciary and payee services, legal services, child care services, housing counseling services. Veterans Grant and Per Diem Housing Program

Grant and Per Diem (GPD) Housing Program The Grant and Per Diem Housing (GPD) Program is a transitional living experience for homeless Veterans in need. Veterans can reside in a GPD program up to two years while working on recovery, employment and independent living skills. GPD has a capacity for 36 Homeless Veterans housed throughout three large houses in Jackson County. This includes a small one-bedroom cottage that can house a single male, single female, couples, and LGBTQIA+. Program admission is contingent upon final approval after screening from VA GPD Liaison and GPD Program Staff. (Jackson County)

Healthcare for Homeless Veterans Housing Program (HCHV)

Valor Pass offers short-term, transitional housing in Jackson County for Veterans who are experiencing homelessness. This program can serve up to 10 Veterans, and staff are available 7 days per week with an onsite live-in On-Site Resident Peer.
 
Various Support Groups and Outside Agency Outreach are offered 3 days per week, and each Veteran will co-author a person-centered “Recovery Care Plan” to outline specific goals for obtaining permanent housing in the future. ColumbiaCare works closely with each Veteran’s VA Case Manager while the Veteran is staying at the Valor Pass Program. All referrals for the program come directly from the local Veterans Affairs Medical Center (VA SORCC). (JACKSON COUNTY)

Veterans Supportive and Long-Term Housing

  • Colonial Estates offers 11 units of Veteran-specific housing in the community. ColumbiaCare acts as a "Veteran-friendly landlord" and partners with various community agencies including the Veterans Administration to coordinate referrals/applications for tenancy.

  • Coronado offers 9 units of Veteran-specific housing in the community. ColumbiaCare acts as a "Veteran-friendly landlord" and partners with various community agencies including the Veterans Administration to coordinate referrals/applications for tenancy.

  • Main Street House is a transitional, shared, affordable housing resource serving Veterans who qualify as low-income.

    Main Street offers 5 individually leased bedrooms, with 2 shared bathrooms, living area, kitchen, and laundry room.

    Tenancy is a month-to-month lease with the intent of each person moving towards individual living arrangements in a time span of about six months. Ongoing case management is available for anyone in need and is strongly encouraged.

    The rental rate varies depending on the size of the bedroom ranging between $420-$600 per month. Utilities are included in the monthly rent. A cleaning deposit equal to one month’s rent is required.

    ColumbiaCare prioritizes Veterans from our GPD and Valor Pass housing programs but will consider any Veteran for tenancy.

  • Royal Court offers 17 units of Veteran-specific housing in the community, 3 of which are shared rentals providing an alternative for Veterans to utilize as transitional, or permanent housing. ColumbiaCare acts as a "Veteran-friendly landlord" and partners with various community agencies including the Veterans Administration to coordinate referrals/applications for tenancy.

  • Sheraton Court offers 3 units of Veteran-specific housing in the community. ColumbiaCare acts as a "Veteran-friendly landlord" and partners with various community agencies including the Veterans Administration to coordinate referrals/applications for tenancy.

  • Wilkes Duplex offers 2 units of Veteran-specific housing in the community. ColumbiaCare acts as a "mental health and Veteran friendly landlord" and partners with the VA in Lane County to coordinate referrals/applications for tenancy.

  • Stewart Avenue Apartments provides permanent, affordable housing specifically for low-income local Veterans. Stewart Apartments has a Onsite Resident Manager who welcomes new tenants, provides on-site presence, helps maintain community spaces, and support tenants in achieving the independence they desire from living in their own safe and habitable units.

    An on-site office, laundry facilities and community room allows tenants to engage and socialize with on-site staff and other residents in social activities, skill-building classes, and fun community activities. (JACKSON COUNTY)
