The ICM team provides community-based services that meet clients where they are – literally. Through a strength-based care lens and team approach, we provide intensive support which is individualized to meet your immediate therapeutic and case management needs.
Upon approved referral, your ICM team will include a therapist, case manager, and peer support specialist who work alongside you to identify and enhance individual strengths, build and maintain connections to community support systems, learn and apply relevant skills, increase self-sufficiency, and foster sustainable change for your overall health and well-being.
A primary goal of the ICM team is to help divert individuals from hospitalization when appropriate and/or step them down from higher levels of care into available sub-acute or community-based treatment.
