Supportive Housing

ColumbiaCare offers a continuum of Supportive Housing programs. All are non-licensed, affordable residential settings that offer varying levels of staff and/or programming, and may be permanent or transitional, based upon the specific program. Some Supportive Housing programs are fully integrated, independent living sites. Our Rental Assistance Programming staff provide housing case management service that help connect individuals to third-party housing units, barrier removal, and ongoing skills training to help with housing success. Aster Place

  • Aster Place is a 2-unit Transitional Housing program for individuals who have a mental health concern and need temporary housing and support services but are not in need of higher levels of care. The individual bedrooms are move-in ready, but tenants must supply their own food, clothing, transportation, personal items, and medication. ColumbiaCare staff provide 4 periodic health and safety visits per every 24 hour period.

Bay Apartments

  • Bay Apartments I offers 10 studio units of affordable, transitional Supportive Housing (up to 18 months) for adults who are living with a mental illness. Bay Apartments II offers 6-units of larger, affordable, transitional Supportive Housing for individuals (and their familles) who are living with a mental illness and can be successful in their housing with minimal supports and community services. The intention of Supportive Housing is to help individuals get back into the regular commercial housing market by providing a transition from houselessness to independent living. Coos County Mental Health provides on-site Case Management services. Funding for these services is provided by Coos Health + Wellness.

  • East Grove is a congregate housing program offering 5 bedrooms on a transitional (up to 60 days) basis for adult Jackson Care Connect members who need extra supports for ongoing stabilization. This program serves as a stepping point between higher levels of care and more permanent, independent living. Staff are on-site during the day, 7 days a week, to ensure stability and provided supports as needed.

Greenburg Apartments

  • Greenburg Apartments offers eight 2-bedroom units of permanent housing for adults living with mental illness who are able to live independently in the community with the proper community-based supports. Greenburg offers some housing supports (1915i), with all other mental health services provided by the tenant’s designated mental health provider. This program helps individuals to manage their mental illness, continue their recovery, and maintain an affordable, independent living situation in the open housing market.

Orchard Home

Pony Creek

  • Pony Creek is a 4-bed program, offering transitional room and board services for adults who have a severe and persistent mental illness. Room and board services are provided 7 days a week and include the following supports: meal preparation, cleaning services, skills training, and medication dispensation. With 24/7 on-call staff available for emergencies.

Rogue Ridge Apartments

  • Rogue Ridge is 28-unit Permanent Supportive Housing program providing affordable housing to local citizens who are low-income and without housing, along with a combination of behavioral health and tenancy supports.

    General staffing includes an on-site Resident Manager available to provide access to housing support; and a Supportive Housing Associate and Certified Recovery Mentor (Peer) providing 7-day a week presence and services such as skills training, tenancy assistance, housing stabilization, and community navigation. The program is supervised by a full-time Supportive Housing Supervisor, and ColumbiaCare's Property Management team offers tenancy support around move in/out, rent collection, and maintenance and repairs.

    Rogue Ridge is a beautiful, three-story complex complete with elevator and ADA-accessible units. There is a community/activity room for residents to participate in supportive services, community and skills-building activities, engage in structured learning, and find social connection. There are multiple community spaces for residents to enjoy, including a sky bridge which connects the buildings and offers beautiful views of the mountains. The grounds include outdoor seating areas and a bike room for storing personal transportation. The building was designed to ensure congruity with the surrounding buildings in the area and fits beautifully into the community. We are proud to share that the project was developed according to Earth Advantage Certification to ensure eco-friendly "green" standards.

    Rogue Ridge is a true community resource, with referrals coming through the Jackson County Coordinated Entry System. Jackson County citizens who meet criteria will have access to this housing based on need. A portion of the units will be designated for Jackson Care Connect and AllCare members.

    Applications for tenancy are not directly available to the public. Eligibility is based upon a vulnerability scale, individuals must meet the HUD definition of chronic homelessness and income requirements. Swing Lane Apartments

  • Swing Lane offers shared units of permanent affordable Supportive Housing to Jackson Care Connect Members living with a mental illness. Varying levels of support are provided based upon need.

    Intensive Supportive Housing Units: Supports individuals in maintaining an affordable independent living situation in the open housing market. Awake staff are onsite 8am-midnight, with on-call coverage all other hours, and the Supportive Housing Supervisor/QMHP is on-site 40 hours a week. Flexible, wraparound services such as case management, skills training, supported employment and other outpatient services are tailored to the individual’s needs, and are available in the community. Clients are supported by on-site programming, which includes daily contact and support from staff.

    Transitional Supportive Housing Units: Services are provided to prepare individuals for more permanent, independent living. Room and board services are provided 7 days a week, which include assistance with cleaning; medication support; some skills training; and 24/7 on-call staff for emergencies.

    All apartments qualify for HUD funding. Tenants are responsible for their rent (or a negotiated portion thereof) and all utilities.

    Applications for tenancy are not directly available to the public. All tenants are screened for clinical appropriateness and income eligibility by their ColumbiaCare service provider and Jackson Care Connect.

Integrated Housing programs offer permanent or transitional housing units that enable individuals to attain and maintain affordable housing within the community. Support services can be offered to tenants living in Integrated Housing and are available as needed or desired, but are not a requirement of tenancy. Other identifying factors of Integrated Housing is no more than 25% of the units in any building can be occupied by individuals with a disability, and housing providers cannot reject individuals for placement due to medical needs or substance abuse history. Non-mental health units are rented as affordable housing to the general public.

Rental Assistance Program

In keeping with our “Housing First” philosophy, ColumbiaCare Services’ Rental Assistance Program (RAP) is designed to help high-needs individuals obtain independent housing in the community. Clients who qualify have a diagnosis of serious mental illness and are stepping down from higher levels of care, such as the state hospital or a licensed treatment facility. These individuals typically face many barriers to obtaining housing and it’s our focus to help them to overcome those obstacles. We provide support from the beginning of the housing search to help our clients see that it is possible to live outside of acute care settings and become a permanent member of the community.

For more information about receiving Rental Assistance Program services, please contact your local Community Mental Health Program, CCO covering regions listed below, or your mental health service provider for eligibility/availability.

  • RAP is staffed by Residential Specialists and Peer Support Specialists. Residential Specialists focus on the nuts and bolts of what it takes to obtain housing, exploring community resources and ascertaining services the client may benefit from while encouraging their growth. The Peer Support Specialists are personally aware of the challenges our tenants face because of their own lived experience with mental illness. They are specially trained to use their stories of recovery to support tenants and help them see their own bright futures. These skilled staff work with the clients to determine their specific housing wants and needs in order to best-tailor our housing search to meet their individual preferences and requirements. Then, we identify potential properties that meet the criteria, reach out to the landlord or property management, obtain the required applications, and work with the tenant to complete them and gather necessary documentation. Once submitted, ColumbiaCare Services advocates for the tenant and alleviates concerns they may have during the process.

  • ColumbiaCare Services staff also assesses the tenant's financial resources and, as needed, will provide monetary assistance with application fees, security deposits and other incidentals through state funding for this project. This is available through a one-time payment meant to ensure that lack of funds is not a barrier to obtaining housing. Whenever possible, our staff also works closely with the tenant's treatment providers to ensure everyone is aware of the goals, progress and supporting the same outcome.

  • Once a tenant is placed in housing, the staff will provide ongoing support and education around what it means to be a good resident and neighbor, how to build rental history and credit, securing and maintaining utilities, accessing community resources and how to locate and utilize public transit systems. Tenants are not required to be engaged in treatment services in order to participate, but they are made aware of what resources and services are available should they be interested in engaging them. By encouraging the tenants to develop and meet their own housing goals, the RAP allows them to discover their place in a safe environment they’ve helped to secure. This teamwork dynamic also promotes the growth of positive relationships and increased independence.
